Privacy Statement

LifeHealth is committed to protecting your privacy and developing technology that gives you the most powerful and safe online experience. This Statement of Privacy applies to all of the LifeHealth Web sites and governs data collection and usage. By using any of the LifeHealth websites, you consent to the data practices described in this statement.

Collection of your Personal Information

LifeHealth collects personally identifiable information, such as your e-mail address, name, home or work address or telephone number. LifeHealth also collects anonymous demographic information, which is not unique to you, such as your ZIP code, age, gender, preferences, interests and favorites.

There is also information about your computer hardware and software that is automatically collected by LifeHealth. This information can include: your IP address, browser type, domain names, access times and referring Web site addresses. This information is used by LifeHealth for the operation of the service, to maintain quality of the service, and to provide general statistics regarding use of the LifeHealth Web site.

 

Please keep in mind that if you directly disclose personally identifiable information or personally sensitive data through LifeHealth public message boards, this information may be collected and used by others. Note: LifeHealth does not read any of your private online communications.

Use of your Personal Information

LifeHealth collects and uses your personal information to operate the LifeHealth Web site and deliver the services you have requested. LifeHealth also uses your personally identifiable information to inform you of other products or services available from LifeHealth and its affiliates. LifeHealth may also contact you via surveys to conduct research about your opinion of current services or of potential new services that may be offered.

LifeHealth does share with third parties aggregated data in which no individually identifiable information is contained. Examples of these reports might include identification of the types of health risks faced by the overall employee population for a particular employer or reports showing the percentage of the population in a particular region that have elevated cholesterol levels.

In the course of their duties, various health information and responses may be viewed by employees or contractors of LifeHealth. Currently, our employees are only given access to your personal information to perform a specific job (e.g., provide you with a lost or stolen password or to answer your direct questions about the screening process or your results). In addition, LifeHealth requires all employees that access member information to sign confidentiality and information security agreements as a condition of employment. Furthermore, all employees are kept up-to-date on our security and privacy practices, and are notified about the importance we place on privacy and what they can do to ensure your information is protected.

LifeHealth will release information contained in financial records and/or medical records, including information concerning laboratory test results, and/or any other related information to:

  • Your insurance company, self-funded or third-party health plan, Medicare, Medicaid or any other person or entity that may be responsible for paying or processing for payment any portion of your bill for services;
  • Any person or entity affiliated with or representing LifeHealth, for purposes of administration, billing and quality and risk management;

LifeHealth does not sell, rent or lease its customer lists to third parties. LifeHealth may, from time to time, contact you on behalf of external business partners about a particular offering that may be of interest to you. In those cases, your unique personally identifiable information (e-mail, name, address, telephone number) is not transferred to the third party. In addition, LifeHealth may share data with trusted partners to help us perform statistical analysis, send you email or postal mail, provide customer support, or arrange for deliveries. All such third parties are prohibited from using your personal information except to provide these services to LifeHealth, and they are required to maintain the confidentiality of your information.

LifeHealth does not use or disclose sensitive personal information, such as race, religion, or political affiliations, without your explicit consent.

LifeHealth keeps track of the Web sites and pages our customers visit within LifeHealth, in order to determine what LifeHealth services are the most popular. This data is used to deliver customized content and advertising within LifeHealth to customers whose behavior indicates that they are interested in a particular subject area.
